Return Merchandise Authorization

If you’d like to send your machine to us for repair, please complete this form. We charge a flat rate of $150 plus parts. After we receive and evaluate your machine, we’ll let you know the cost of repair, and at that point we’ll request payment. If you choose not to proceed, we will charge a $75 bench fee. Please request one RMA number for each machine you want to send in.
